What insurances do you take?

At Proof Physical Therapy and Performance, we take many different types of insurance in order to serve a wide variety of patients. The list is being updated frequently, so check back or call if you have any questions. Many insurance plans have out of network benefits, so if you don’t see your insurance company listed, don’t be afraid to come in! We accept cash payment and are currently in-network with the following providers:

  • UMR
  • Tricare
  • Medicare
What is my co-pay/co-insurance cost?
Each insurance plan has a different set of benefits for it’s providers. This means that your co-pay or co-insurance depends on which plan you have. For our in-network patients we are usually able to look this information up for you. Even if you have a co-worker who has the same plan, your cost of care may be different due to different deductible amounts, different plans chosen, etc. We strive to be as upfront as possible regarding your cost and welcome any questions you may have.
What is a deductible?

 A deductible is the amount you pay for covered health care services before your insurance plan starts to pay. With a $2,000 deductible, for example, you pay the first $2,000 of covered services yourself. After you pay your deductible, you usually pay only a copayment or coinsurance for covered services.

Who can I contact for information about my insurance?
The best place to call is your insurance company for questions regarding your benefits. Many insurance companies have websites where you can check your benefits as well. Below is a list of some of the insurance companies that we are in network with and their contact information. If you call and get information that you don’t understand, feel free to call us and we can help.

What is your financial policy?
At Proof Physical Therapy and Performance we put our patients first. We recognize that insurance can be a confusing situation and are glad to help our patients understand it as much as possible so they can focus on getting great care, instead of worrying about finances. It is our policy to collect co-pays at each visit, to collect balances due on a regular basis, and to keep the patient updated about insurance progress or changes when the information becomes available to us. If there is a discrepancy or a question about the billing, insurance, or anything relating to these you are more than welcome to address these with one of our staff.
