 When having physical therapy or training services, as appropriate, you may be asked to enter your name, email address, mailing address, phone number, insurance, date of birth or other details to help you with your experience.

 We collect information from you when you contact our office to make an appointment for physical therapy or training. Additionally, we collect information on your first visit.

How do we use your information?

 We may use the information we collect from you when you register, sign up for our newsletter, respond to a survey or marketing communication, surf the website, or use certain other site features in the following ways:

  • To personalize user’s experience and to allow us to deliver the type of content and product offerings in which you are most

How do we protect visitor information?

 Our website is scanned for malware and viruses on a regular basis, including for security holes and known vulnerabilities, to make your visit to our site as safe as possible. Proof PT does have an SSL certificate on it’s website which increases the level of security. The security of your Personal Information is  important to us but remember that no method of transmission over the Internet, or method of electronic storage, is 100% secure. While we strive to use commercially acceptable means to protect your Personal Information, we cannot guarantee its absolute security.

Fair Information Practices

 The Fair Information Practices Principles form the backbone of privacy law in the United States and the concepts they include have played a significant role in the development of data protection laws around the globe.
