About Proof Physical Therapy and Performance
Most clinics manage your symptoms and call it a plan. Manage them long enough and they quietly become the plan. We opened in Flagstaff in 2016 to end them instead: one-on-one with a licensed physical therapist, every visit.
Our Story
Dr. Jordan Williams, DPT, Cert DN, Founder
I earned my B.S. in Physiology at the University of Arizona. Then I worked as a PT tech in Tucson, where I picked up Spanish well enough to treat in it. I graduated from the Doctor of Physical Therapy program at Northern Arizona University in 2014. My doctoral capstone was on diagnosis and treatment of concussion in the physical therapy setting.
I opened Proof in Flagstaff in November 2016 because I had seen the other model up close. Patients stacked up, a tech running the exercises, a printout at the door. So I built the opposite. One person, one plan, and a straight answer about what is actually causing the pain instead of a list of the places it hurts.
Since then I have built out a cyclist assessment program, trained as a Level 2 Running Gait Analyst through RunDNA, and worked with local teams through their off seasons. Away from the clinic I am on a mountain bike, playing bass, in the woodshop, or camping with my wife and our three kids. None of that is the point of this page. You are. The people who do best here are the ones who fix things: their houses, their teams, their families. This is where they finally fix the thing they’ve been putting off: themselves.

You shouldn’t need a punch card to stay out of pain. Your plan has a finish line, and you’ll always know how close you are to it.

The Difference
One less hurdle: in Arizona you don’t need a physician referral or a prescription to start physical therapy. Medicare, Medicaid, and a few other plans do require one. Otherwise, you can just call us.
