Weeks 1–2
Pain
Calm the pain down.
Hands-on work settles the irritated tissue and takes the edge off, starting your first visit. Manual therapy, dry needling, and spinal manipulation are all on the table if they fit your back.
What We Treat
Standing up straight without bracing on something. Sleeping through the night. Lifting your kid without doing the math first. That starts with a cause-first evaluation instead of a guess, one-on-one with a licensed physical therapist.
Sound Familiar?
Two or more? Keep reading. You’re the one everybody else leans on. That’s exactly why you’re the last one to get looked at, and exactly why you’re still reading.
Why It Happens
Your back doesn’t hurt for no reason. And it’s rarely just one thing. Usually it’s some mix of four: a nerve that’s irritated and referring pain down your leg. Spinal joints that got stiff and quit sharing the load. Deep support muscles that quietly went offline after your last flare-up. And the daily stuff, how you sit, bend, and lift, reloading the same spot before it ever heals.
None of that shows up in a glance at your posture. It takes a real evaluation to sort out which one is actually driving your pain. That’s why rest, ice, and a printed stretch sheet so often fall short. They aim at the symptom, not the source. Find the cause, and the fix stops being a mystery.
The Honest Truth
Nobody regrets starting too early. Every week, someone in our clinic says they regret waiting.
How We Fix It
Pain, Prime, Perform, Prevent. Each phase has a job and a way to know you’ve earned the next one. One-on-one with a licensed physical therapist, every visit.
Weeks 1–2
Pain
Hands-on work settles the irritated tissue and takes the edge off, starting your first visit. Manual therapy, dry needling, and spinal manipulation are all on the table if they fit your back.
Weeks 2–6
Prime
Targeted mobility work gives back the motion in the segments that have been guarding since the injury. Bending stops being a negotiation.
Weeks 6–10
Perform
Progressive core and hip strengthening builds the support your spine has been missing. This is the phase where the flinch goes away.
Weeks 10+
Prevent
Lifting, bending, and twisting get tested under real load before you’re cleared. You leave with the work that keeps it gone. That’s the endpoint, not another appointment.
Questions, Answered
Usually not, and it’s rarely the first step. Most back pain gets better without imaging. Plenty of people with no pain at all have disc bulges and wear and tear on their scans, so a picture doesn’t always name the cause. We start with a hands-on evaluation to find what’s actually driving your symptoms, and if imaging turns out to be necessary, we’ll help you get there. One more thing worth knowing: in Arizona you don’t need a physician referral to start physical therapy. Medicare, Medicaid, and a few other plans do require one, and we’ll tell you straight if yours is one of them.
In most cases yes, and movement usually helps more than rest. Hurt doesn’t always mean harm. Total rest tends to leave a back stiffer and weaker, which sets up the next flare-up. The trick is dosing the right movement for your back specifically. That’s what a licensed physical therapist sorts out at your evaluation, then progresses as you get stronger. You will not be handed a sheet and left to guess.
You’ll get an honest answer at your evaluation, once someone has actually examined your back. What we won’t do is leave it open-ended. Your plan runs through four phases, Pain, Prime, Perform, and Prevent, and each one has a job and a way to know you’ve earned the next. You’ll always know which phase you’re in and what’s left. That’s a plan with an endpoint, not an open tab of appointments.
