Privacy Policy

This policy covers proofpt.com, the website of Proof Physical Therapy and Performance, 460 N Switzer Canyon Dr #400, Flagstaff, AZ 86001. It describes the information this website collects and how we handle it.

This policy is about the website. It is not our Notice of Privacy Practices under HIPAA. Protected health information created in the course of your care is governed by that separate notice, which we provide at your first visit and will send you on request.

Information you give us

This site has three forms. You choose whether to use them, and each one tells you what it is for before you submit it.

• Appointment request. Your name, phone number, email address, and what you tell us about your pain: the area, how long it has been going on, and how it is affecting you. We use this to contact you and to prepare for your first visit.
• Free benefits check. Your name, date of birth, ZIP code, insurance provider, member ID, group number if you have one, and your phone and email. We use this only to verify your coverage with your insurer and to tell you the result.
• Pain Check. A short set of answers about where it hurts and how long. If you complete it without submitting the booking form, we receive the answers without your name or contact details attached.

The benefits check collects an insurance member ID. That is sensitive. We transmit it over an encrypted connection, we use it only to check your benefits, and we do not sell it, rent it, or share it for advertising.

Information collected automatically

Our hosting provider records standard server request data such as IP address, browser type, the pages requested, and the time of the request. This is used to keep the site running and secure.

Third parties this site uses

• Netlify hosts the site and processes form submissions.
• Google Maps provides the embedded map on our contact and location sections. Loading that map involves a request to Google.
• Elfsight displays our public Google reviews. The reviews shown are already public on our Google Business Profile.
• Google Fonts serves the typefaces used on this site.
• Our practice management and patient communication systems receive appointment requests so our front desk can respond to you.

Each of these providers has its own privacy policy governing what it does with data it receives.

Text messages

If you check the consent box on the appointment request form, you agree to receive text messages from us about your appointment. Message and data rates may apply. Reply STOP to any message to opt out, or HELP for help. Consenting to texts is optional and is not a condition of receiving care. We do not sell or share your phone number with third parties for their own marketing.

How long we keep it

Form submissions are retained as long as needed to respond to you and to meet our recordkeeping obligations, then deleted. Clinical records are retained under the schedule required by Arizona law and by our HIPAA notice, not this policy.

Your choices

• You can use the site without submitting any form.
• You can ask us what website information we hold about you, and ask us to delete it, by emailing info@proofpt.com.
• You can opt out of text messages at any time by replying STOP.
• Your browser can block or delete cookies and can send a Do Not Track signal. This site does not use tracking cookies for advertising.

Children

This website is not directed at children under 13, and we do not knowingly collect information from them through it. We do treat minors in the clinic; in that case a parent or guardian provides their information directly to us.

Changes to this policy

If we change how this site handles information, we will update this page and the date at the top of it.
